Admission Application for School of Early Education

Dear Parent/Legual Guardian,

Please be sure to review all steps and documents required to complete this online application form. Upon receipt and review, Holy Angels administration will send notification regarding assessments and interviews. Thank you for considering Holy Angels School for your child’s education. REQUIRED DOCUMENTATION FOR PK, TK, and KINDERGARTEN APPLICANTS

Copy of birth certificate. If you do not have this document and your child was born in Los Angeles County, contact the County Registrar of Records at (562) 462-2137.
Copy of Catholic Baptism if applicable. Contact the Church of Baptism if you do not have the original document.
Transitional K/Kindergarten teacher recommendation form to be completed by your Child's current teacher.
NO application will be processed until ALL the documents are presented.

APPLICATION FEE: $75

FOR SCHOOL OF EARLY EDUCATION (PK, TK, and K)

PK applicants must be 3 years of age and potty-trained by September 1.
TK Applicants must be 4 years of age by September 1.
Kindergarten applicants must be 5 years of age by September 1.

EARLY EDUCATION ASSESSMENTS

All PK, TK, and Kindergarten applicants must demonstrate developmental readiness, and will be subject to Early Childhood Director’s recommendation.
Students applying to our PK, TK, and K will be invited into classrooms to meet with and interact with teachers. These assessments will take approximately 20 minutes.
Registration for assessment can only be made when completed applications, including all required recommendations and documents have been submitted

TO THE PARENT/LEGAL GUARDIAN: As part of the admissions process at Holy Angels School we must receive a candid assessment of the applicant. The student’s application will not be processed without this completed form. Please email the following link to your student's teacher: Confidential Teacher Recommendation Form. He or she will appreciate being given plenty of time to complete this form. Failure to complete this file will result in your child losing admission preference.
