1. By submitting this form, parents agree to pay all daycare fees promptly and fully.
2. Checks should be made out to Holy Angels School.
3. Fees are due and payable on the first school day of each month.
4. No child will be allowed to remain in daycare if the family is more than two months delinquent with payments unless special arrangements have been made with the Daycare Director and the School Principal.
5. Consistent late pick-up may result in your child being asked to leave daycare.

 Registration Fee for Full-time Daycare:     $ 75.00 per student
 Enrollment Billing Codes:                           

A =  A.M. Daycare: (7:00 a.m. – 7:30 a.m.)
B =  P.M. Daycare:  (3:00 p.m. up to 6:00pm)
C =  A.M./P.M. Daycare:  (7:00 a.m. – 7:30 a.m. & 3:00 p.m. up to 6:00 p.m.
D =  Mid-Day Daycare (Pre-K/JK Only):  (11:30 a.m. up to 3:00 p.m.)

Registration Fee for Part-time Daycare:      $ 50.00 per student
